Everything you need before your first visit

A little preparation makes the first appointment go smoothly. Here's what happens, what to bring, and what to expect on the billing side.

Your first appointment

What happens on day one

A 60 minute appointment, spent with a licensed physical therapist.

  1. Arrive about 15 minutes early so we can complete paperwork without cutting into your appointment.
  2. Your therapist will talk with you about your history, your symptoms, and what you want to get back to.
  3. You'll go through a hands-on examination of movement, strength, joint mobility, and — where relevant — balance or nerve-related symptoms.
  4. Your therapist will explain what they found in plain language and what it means for your plan.
  5. Together you'll set goals and agree on a visit schedule and a home program.
  6. Treatment begins on the first visit, so patients leave with something to work on and clear understanding of the plan of care, and treatment expectations.

What to bring

  • Photo ID
  • Your insurance card (and secondary insurance, if you have one)
  • Any physician referral or prescription for physical therapy
  • Imaging reports or surgical notes, if you have copies
  • A list of current medications

What to wear

  • Clothing you can move comfortably in — nothing restrictive
  • Shorts if you're being seen for a knee, hip, or ankle problem
  • A tank top or loose t-shirt for shoulder or neck concerns
  • Supportive athletic shoes, if you have a pair
  • For work injuries, footwear similar to what you wear on the job

Forgot something? Not a problem — we keep a small supply of shorts and gowns at the clinic.

Before you come in

Referrals, forms, billing, and policies

Do you need a referral?

Washington State allows direct access to physical therapy, so in most cases you can be evaluated without a physician referral. Some insurance plans still require one for coverage, and a few limit how many visits are allowed before a referral is needed.

Call the clinic before your first visit and we’ll check your specific plan so there are no surprises.

New patient forms

There’s nothing to download here. Once your first appointment is scheduled, we’ll send your new patient forms to you digitally, set up for your specific visit. Completing them before you come in means more of your appointment is spent on care rather than paperwork.

If they haven’t arrived a day or two before your appointment, give us a call and we’ll resend them. You can also simply arrive 15 minutes early and complete them at the clinic.

Insurance and billing

Most plans cover physical therapy, but deductibles, copays, coinsurance, and visit limits vary widely. We’re glad to help verify your benefits before you start.

If you don’t have insurance, ask about our self-pay rates — we’ll tell you what an evaluation and follow-up visits cost up front.

Cancellation policy

If you need to cancel or reschedule, we appreciate at least 24 hours’ notice when you can manage it — that way we can offer the time to another patient who is waiting.

We know life doesn’t always cooperate, and there’s no fee for a late cancellation. Just give us a call and we’ll find you another time — we’d much rather hear from you and get you rescheduled than have you miss out on your care.
